Sports Physical Night Information

Sports Physical Night Information

Here is some information to help your daughter get started participating in Athletics. Every student athlete must have a current physical (After June 1) and Final Forms registration completed prior to participation. Below is the information for both Final Forms registration and Sports Physical Night on campus. We will also have presentations on Collegiate Athletics Recruiting 101 and preseason meetings for basketball and cross country. The Volleyball Open Gym dates for the summer are also listed. If you have any questions, please do not hesitate to contact us.

Final Forms Registration

In order to participate, you need to register your child on Final Forms (https://stfrancishs-ca.finalforms.com/).

When you log in, the system will still be set for 2018-19.  In order to switch to the 2019-20 school year, you will need to click on the blue box at the top of the screen (see below).

Once you have switched to the 2019-20 view, you will need to click the Add Sport next to your child's name.  After adding a sport, it will take you back to the My Student view (see below). Click on the blue Incomplete Forms button to review or complete the necessary forms. Once you have completed the forms, your child will be emailed to sign as well.

St. Francis Sports Physical Night - Wednesday, June 5, 2019 5-8 p.m.

St. Francis will be hosting Sports Physical Night for the 2019-20 school year on June 5 with registration starting at 5 p.m.  The cost of the physical is $25 per student. It is payable with credit card or check or can be prepaid online. If you have been grandfathered in as a lifetime Booster member your physical is free if you present your membership card.

We will have an army of Troubie parents who volunteer their time to complete the yearly required sports physical your child must have to participate in Athletics. While this event is not mandatory to attend, it is a great opportunity to get the required task out of the way for the 2019-20 school year. Students will go through a full "well child" pre-participation exam administered by a licensed physician.  Before participating in the exam students will be asked to fill out a health history questionnaire and have their vitals measured.

Please be aware, if you are not accompanying your child to the event you must complete and sign permission slip in advance as well as sign the bottom of the health history on the physical form. Both forms can be found below.

The Troubie store will be open during the event and Chromebook pick up will be held in the Technology Department.

We will also be offering Diocesan-approved fingerprinting services ($45) in the Faculty Lounge. If you plan to drive other students to athletic events or volunteer on campus this is the cheapest opportunity to have your fingerprints done for the Diocese.  Please note, fingerprinting only needs to be completed once during your time as a St. Francis parent and if you were fingerprinted for a diocesan elementary school you do not have to be re-printed.

If you choose to have your physical completed by your doctor please note that all exams must take place after June 1 and forms are due to the Athletic Department before participating in a Fall Sport and/or summer conditioning. Please have your physician complete the St. Francis form found on our website or at the link listed below. If your daughter has a preexisting known medical condition it would be a good idea to schedule her physical with her personal physician.
